Was unemployed from 10/27/04 to 2/14/05 due to FMLA running out at Health Care Logistics. I fell on 7/9/04 dislocating and breaking my right ankle requiring surgery with the insertion of a plate and twelve screws. I was in a cast until 9/20/04. Due to atrophy in my foot / ankle I required extensive physical therapy and was not released to return to work until 11/16/04. I drew unemployment from that point until 12/13/04 when I worked for about a month in a Temporary Position as a QA Tech for American Health Packaging. *** Note: The injury was not sustained on the job at Health Care Logistics.
